Admissions
The determination of appropriate and least restrictive services for the adolescent is made through a process of referral material, communication with the referring agency, and an intake interview with the resident and her parent(s) whenever possible. Referral information should include as much of the following as possible:
• Court Order
• Offense History
• Social History
• Prior Placement Summaries
• Recent I.Q. Scores
• Immunization Records
• Psychological Evaluations
• Psychiatric Evaluations
• Educational Records
• Third-party Insurance Information
• Social Security Number
• Birth Certificate
